In order to safely and securely travel with our students it is important that these forms be filled out completely and returned to your child's music teacher. All of the necessary forms can be downloaded here if the originals that we sent home have been lost. You can also find more information on each form here.




LMSD requires all students traveling to have a Consent Form (LM32) on file. This form gives us the security that a student's parents/guardians have authorized them to travel with us.
LMSD requires all students traveling to have a Medical Form (LM29) on file. This form must be filled out and returned even if you are not authorizing any medication to be dispensed. As a reminder, students are not permitted to carry any medications themselves except as indicated on the Medical Form. Similarly, LMSD nurses cannot dispense any medication that is not listed on the form, no matter how trivial it may seem. Please err on the side of caution and list every medication you think your child may need while on the trip.

Code of Conduct Form




The LMHS Music Department requires all students traveling to have a signed Code of Conduct form on file. This is a school-sponsored trip and is viewed as an extension of LMHS. All school rules and policies must be adhered to on this trip. The Code of Conduct Form serves to reinforce this. Violations of the Code of Conduct are taken very seriously and can result in students being sent home at their own expense. (It has happened before.) Serious infractions will be handled by the LMHS administrator on our trip (Mr. Kilpatrick).
If your child requires medication to be administered while on the trip the appropriate form must be completed and returned to the LMHS Nurses' Office.
  • If your child will require over-the-counter (OTC) medication, fill out and return form LM28c. A separate form must be submitted for each medication. OTC medications still require a physician signature. The only exceptions are Tylenol and ibuprofen.
  • If your child will require prescription medication a separate form LM28c (including physician signatures) must be submitted for each medication.
  • If your child needs to be authorized to carry an emergency medication (e.g. an Epi-Pen) on their person, fill out and return form LM28d.
  • If the school already has these forms on file for your child you do not need to submit them again. This is commonly the case with emergency medications, but may also occur with other medications.
